何人家族ですか (nan nin kazoku desu ka) |
何人家族ですか (nan nin kazoku desuka?) is used to ask other people how many people there are in their family. When replying, one must also count themselves. For example, there are four people in Johnson’s family, so she would answer 四人家族です。
何人兄弟ですか (nan nin kyoudai desuka?) is used to ask other people how many siblings there are in their family. When replying, one must also count themselves. For example, there are just Naomi Johnson and her brother, two siblings altogether, so she would answer 二人兄弟です。

In English, one would say, “I have a brother.” or “I have a sister.” In Japanese, it is much more common for one to specify if they have an older brother, younger brother, older sister, or younger sister.
